New 4-H families - never, ever in Ohio 4-H in any Ohio county - are to complete and return a paper enrollment form to their advisor or Extension office. New enrollment forms can be received from your club advisor, or if you prefer, click the link below to complete the pdf - including a parent/guardian signature - and return to your advisor or Extension office by the enrollment deadline. All information is required on the enrollment form.

If someone in your family was involved in Ohio 4-H from 2015 through September 2022, you already have a family profile in 4-H Online. Extension staff will create your personal member profile from the information provided on the enrollment form.
