New to 4-H?

New 4-H families are to complete and return a paper enrollment form to their advisor or Extension office. New enrollment forms can be received from your club advisor or the Extension office. If you prefer, click the link below to print the form, then return it to the Extension office (during business hours or the locked drop box available 24/7). All information is required on the enrollment form, including a parent/guardian signature. Extension staff will create your personal member profile from the information provided on the enrollment form.
