New to 4-H?

New 4-H families are to complete and return a paper enrollment form to their advisor or Extension office. New enrollment forms can be received from your club advisor or the Extension office. If you prefer, click the link below to complete the pdf, print, and return it to the Extension office. All information is required on the enrollment form, whether paper or pdf, including a parent/guardian signature. Extension staff will create your personal member profile from the information provided on the enrollment form.
